Any product of Herb Soul Alchemy is not intended to diagnose, treat, cure or prevent any disease.
I understand that a Wellness Consultation is not a substitute for medical care by a licensed physician or other medical provider.
I understand that Herb Soul Alchemy is not a licensed health care practitioner or medical doctor and cannot prescribe any medications.
I am advised and agree to consult with my physician before beginning any new Wellness protocols.
I understand that Wellness product may interact with prescribed or over the counter medications and I agree to disclose all medications that I am currently taking. I agree to consult my physician before starting any new medications.
I understand that I am seeking this consultation solely for educational/entertainment purposes and my own informed-decision making. I am not obligated to follow any recommendations made and assume the risk of any potential interactions.
I agree that I am solely responsible for my decisions and actions based on the information that I receive.
I understand that any and all disputes, controversies, claims or demands arising out of or relating to this waiver, the providing of services by Herbal Soul Alchemy shall be resolved by binding arbitration administered by the American Arbitration Association under its Commercial Arbitration Rules then in effect. Any such arbitration proceeding shall be conducted in Bexar County, Texas. Binding arbitration means that no party to this Agreement can file a lawsuit against the other (except to compel arbitration) and that all disputes or claims arising out of this Agreement or the legal services provided will be submitted to arbitration where there will be no jury or appeal to the courts.
Payment
Payment is due at the time of booking.
